What should you know about this plan?
- This is a dental discount plan, NOT dental insurance nor a qualified health plan. It is not combinable with insurance nor other offers or discounts. It is also not retroactive, transferable, or refundable.
- This plan does not include treatments or procedures completed outside of our office.
- Patient account must be current to receive services & discounts. Participant must remain a plan participant for entire duration of treatment to retain discount.
- Rates subject to change
- Effective date is the day you sign up and your auto renewal date is the same date every year, all services must be rendered before the end coverage date.

- The initial six months of membership fees are due in full before membership activation. A credit card is required for ongoing charges. After the first year, membership will automatically renew.
- Estimated values are based on 80% of standard prices in our area for the included procedures.
